How about the 6530?  Here's what I have for DOS 1 (originals) 

  2532 - 901468-06 (date codes 1679 and 3379)
  2532 - 901468-07 (date codes 1479 and 3379)
  6530 - 901466-02 (date codes 1079 and 3179)

My V2.x sets are from 1981 and 1983 and contain 901468-14, 901468-15, 901468-16
and 901466-04.

Not at the moment.  I would have to write one (or stick my ROMs in my
ROM programmer possibly with a pin swapper)

My 2040 drives all have V2 of CBM DOS.  My V1 chips all came out of the
spare parts bin of the local Commodore dealer when I bought his service
department as scrap almost ten years ago.  I could see about reading these
in, but I suspect that they are identical to what's on funet.
