do you know the part number of your DOS ROM chips? Is it 318045-01 or
318045-02?

The -01 have a bug in that they "forgot" an SEI, meaning that an error
interrupts the reading process.

What you experiences looks very much like you were bitten by exactly the
bug fixed in the -02 ROM.
