The video output of C64 and Plus/4 are the same, you can use the same 
cable for both.

Pin 3 ist Audio out, Pin 5 is audio _in_. You want to use pin 3

The type of plug you need is known as MAS80S.

If you want to use S-Video instead of Composite, then you need to use 
pin 1 (luma) and 6 (chroma) on the Commodore side.
