I believe your unidentified PET CP/M board is the "Z-RAM" from Madison 
Computer.  There is a review of the Z-RAM on page 91 of Commodore Magazine, 
April/May 1982 issue.  The review includes a photo of the board and it looks 
like it matches your photo.
