I'm happy to whip up a PCB design in an hour or so, if Ruud will send 
the EAGLE SCH file (it looks definitely like EAGLE was used).  But, I 
only use outside firms for etching.  I am happy to submit an order on a 
cost recovery basis for a small batch of boards, if someone else will 
ship them out to folks (I'll do the paperwork and submit the order, have 
them shipped to someone else for distribution)
