There is one double-sided disk, with 8050 format on one side and 4040 format 
on the other.  I recently added an image of the 8050 side to my page.

Each disk side contains PET programs and also a CP/M filesystem contained in a 
relative (REL) file.  Only one disk is required to boot CP/M.
