As far as the North American version (128DCR, metal case) goes, I have several 
and none of them came with a fan mounted inside.  I don't believe Commodore 
ever included the fan.

There are vents in the rear of the unit, holes for mounting, and the power 
supply has a convenient place to tap 12VDC that looks like it was made for the 
fan.  I added a fan to one of mine years ago.
