When my 8250 powers up, it sometimes flashes all 3 LEDs that way.  It's
usually after I have not used it for a long time.  After it has warmed up,
I can turn it off and back on and it works.  I have not been able to find
a cause other than the possibility of dirt or moisture in the circuits.

The one LED that is different may be connected backwards.  It's easy to
reverse the connection where the LED is connected to the wiring.

The fact that the LEDs flash indicated that the main processor is
functioning.  Not being able to read the command channel probably means
that it did not complete the power up sequence in the ROM.
