Could be that the EPROM is just a tad too slow for the timing used in 
40col mode which seems to differ from 80col mode as you found out.

I had a similiar problem once, in order to use a 2532 as a char-ROM in a 
C64, I had to get 2532-30 (300ns). Anything slower would result in a 
snow storm.
