Sorry for the delay... This is a tidbit I had in my PET FAQ, hope it helps:
Disk Problems:
- If the disk drive flashes a number of flases repeatedly it is reporting
an internal hardware fault, here is a chart:
flashes: 4040 8050

Since 5 seems to be zero page on both the 4040 and 8050 it might be so on an
